AIDS – the modern dreaded disease that has transcended all barriers of age, sex, creed, religion and geographical boundaries. AIDS is the acronym for Acquired Immuno Deficiency Syndrome. Acquired means it is infectious, Immune Deficiency means a weakness in the body's system that fights diseases, and Syndrome means a group of health problems that make up a disease.

HIV - Human Immunodeficiency Virus has been established as the underlying cause of AIDS and the term AIDS applies to the most advanced stages of HIV infection. The virus progressively destroys the body's ability to fight infections and certain cancers by leading to the destruction and/or functional impairment of cells of the immune system, notably CD4 positive T cells.

Origins
It is believed that the Human Immunodeficiency Virus "crossed over" into the human population from a particular species of chimpanzee. The virus has existed at least since 1977-1978 in the United States, Haiti and Africa. AIDS was first recognized in 1981 and has since become a major worldwide pandemic. The Centers for Disease Control and Prevention, Department of Health and Human services, United States Government officially named the condition AIDS (Acquired Immune Deficiency Syndrome) in 1982. The virus responsible for weakening the immune system was identified as HIV in 1984.

The World Health Organization (WHO) in 1988 announced December 1 as the World AIDS Day and every year since then it is observed worldwide and used as a platform to create awareness about AIDS.

The red ribbon became an international symbol of AIDS awareness during 1991. The organization Visual AIDS in New York, together with Broadway Cares, and Equity Fights AIDS, established the wearing of a Red Ribbon as something that signified support for people living with HIV/AIDS.

The new United Nations (UN) Joint Program on AIDS (UNAIDS), brought together six agencies with in the UN system - WHO, UNDP, UNICEF, UNFPA, UNESCO and the World Bank. The UNAIDS program became operational on January 1st 1996 and replaced the individual programs of all these agencies. Later UNHCR, WFP, ILO and UNODC also joined this program brining the number of agencies to 10.

In 2003, almost five million people became newly infected with HIV, the greatest number in any one year since the beginning of the epidemic. At the global level, the number of people living with HIV continues to grow - from 35 million in 2001 to 38 million in 2003. In the same year, almost three million were killed by AIDS; over 20 million have died since the first cases of AIDS were identified in 1981.

AIDS in India
India's socioeconomic status, traditional social norms, cultural myths on sex & sexuality, large-scale migration and a huge population of marginalized people make it extremely vulnerable to the HIV/AIDS epidemic. Since the first case was reported in 1986, HIV has spread rapidly from urban to rural areas and from high-risk groups to the general population. Currently HIV prevalence is estimated at around 0.8% of the adult population (between 15 and 49 years of age). However due to the large population size of over 1 billion, India has a large number of people living with HIV/AIDS, second only to South Africa.

Six out of the 35 States and Territories of India, four in southern India (Andhra Pradesh, Tamil Nadu, Maharashtra, Karnataka) and two in north eastern India (Manipur and Nagaland) have generalized epidemics and account for nearly 80% of all reported AIDS cases in the country. In the southern States, heterosexual transmission accounts for majority of the reported cases, whereas in the north-eastern States, injecting drug use is the predominant mode of transmission.

In 1987, the Government of India established a National AIDS Control Program (NACP) under the Ministry of Health and Family Welfare. Program activities covered surveillance, screening of blood and blood products, and health education. In 1992, with the support of the World Bank, the Ministry established the National AIDS Control Organization (NACO) to coordinate an enhanced Program of preventive activities. NACO provided national leadership and facilitated the development of State AIDS Control Societies (SACS) in all states and union territories across India under a Five -Year Strategic Plan for HIV/AIDS prevention. NACO was able to make a number of important improvements in HIV prevention such as improving blood safety. There are 140 centres to strengthen surveillance and 180 sentinel sites to monitor HIV trends and the geographical spread of HIV among the general population at-risk groups.

The first national convention of elected political representatives on HIV and AIDS was held in New Delhi in July 2003. It was organized jointly by NACO and UNAIDS and brought together 1200 representatives from districts and states all across the country. The national antiretroviral program was launched in April 2004.

There were an estimated 3.8 - 4.6 million persons living with HIV/AIDS in India by the end of 2002. Of these, 600,000 are in need of antiretroviral therapy (ART) but less than 30,000 are currently receiving it. The national ART program will be scaled up as human and financial resources become available.

With a high prevalence of TB infection in India, the problem of TB related to HIV infection also poses a major public health challenge; particularly in the high HIV burden States. The exact magnitude of HIV among TB patients is unknown as routine surveillance of HIV among TB patients in not undertaken.

AIDS and Philately
The reach of the postage stamps is beyond mass media and several governments have shown their support in the fight against HIV/AIDS by putting prevention messages on postage stamps, postal stationery, and postmarks. World’s premier organization of topical stamp collectors, American Topical Association’s check list of stamps issued on AIDS runs in to 14 pages.

Even though India Post has not issued any stamp on AIDS yet, they have issued over 70 items of postal stationery such as postcard and inland letter card since 1997. In addition special postmarks have also been provided by India Post.
